Frequently Asked Questions
Do you accept insurance?
Nutrition services at Horizon are considered out-of-network with insurance providers. Our Dietitians do not accept insurance and are unable to file insurance claims.
Superbills: Upon request, a superbill can be provided to you after your payment is processed. This allows you to submit a claim for possible reimbursement to your insurance provider. You will need a diagnosis given by a medical provider in order to create the superbill. We use procedure codes 97802 for intitial assessment and 97803 for nutrition follow up sessions. Please call your insurance company to understand what coverage or reimbursement may be given.
How do I pay for services?
Payments are due at the time of the service. We accept credit card, checks, cash, flexible spending accounts (FSA), and Health Savings Cards (HSA). Cards are kept on file through your client portal. For our 3-6 month packages, your card will be auto-drafted on the same date each month. For pay-per-session services, payment is due at the time of the session.
What is your Cancellation & No show policy?
We ask for a 24 hour notice by phone or email if your appointment needs to be canceled or rescheduled. No-shows and late-cancels will be charged the full session rate will be auto-charged. Emergencies and inclement weather will be considered. If you cannot make your in-person session, you have the option to do a virtual or phone session vs. canceling the session.
How often do you meet with clients?
Freqency of sessions are based on what you are being seen for. Eating disorder treatment and behavior change is a journey. We typically see clients weekly to bi-weekly to fully support individuals and/or families. There are many variables we consider when making this recommendation including the complexities of eating disorders, co-occuring disorders, mood disorders, level of malnutrition, medical diagnosis, and environmental and social dynamics.
I don't have an eating disorder, can I still work with you?
Yes! We don’t treat “labels”, we treat people. Eating is complex, beliefs and experiences with food are complex and unique. Disordered eating, diet-culture, poor body image, food confusion, etc. are all worthy of healing, you are worthy of healing and feeling free around food and body image.
